The Probate Task Force is currently revising the current Colorado Probate Code (CPC) forms. Once a CPC form is revised and ready to be reissued, it will be converted to a Judicial Department Form (JDF) and will be assigned a number between 700 and 1000 (CPC forms are numbered from 1 to 61). Until January 1, 2009, both CPC and JDF forms may be used. However, it is recommended that JDF forms be used as soon as they are available on this website. As of July 1, 2009, use of JDF forms will be mandatory and CPC forms will no longer be permitted for use in Colorado courts. If you cannot find a JDF form that fits your situation, you are still required to use the applicable CPC forms. If there is not a JDF or CPC form that fits your situation, there is not a standard form available for use that is approved by the Colorado Supreme Court at this time. These forms can be downloaded from the court's website by clicking here.
